Is there a standard form I can get to add to my emails to tell the receipient they can opt-in for more information or opt-out of receiving further information from me?

I am not aware of a standard form that can be used for emails. However, you can use SIGNATURES to add a message automatically to the end of each email.
Go to “Options” on the right side of your email page
Click on “Mail Options”
Click on “Signature”
Enter the signature you want to appear or any message
Click on “Add Signature to all outgoing messages ”
Click on “Save”
